Mandatory: Physical Examinations and
Sports Clearance Physical examinations are REQUIRED for all students
participating in interscholastic sports. A private physical with
your own family physician is acceptable, and is highly
recommended. Your family physician is in the best position to
appraise your child's health, since he/she is most familiar with
your child's medical history.
The
physical examination forms you will need to take to your family physician
now are available online. Click on the appropriate link below to download the form you'll need.
The documents are in PDF format, which requires Adobe Acrobat
Reader to view. Acrobat Reader can be downloaded for free from
the
Adobe Acrobat Web site.
All private physical forms are reviewed by the school nurse
before students can practice.
Want to play a fall sport?
Students must have an up-to-date physical on file in the
Schalmont Health Office, have their parents complete an Interval
Health History Form (unless physical is completed within 30 days
prior to first day of practice), and come to one of the sports
clearance dates (see below) with their paperwork completed.

Fall 2008 Sports Clearance Dates (nurse only):
- Schalmont High School (grades 9-12):
Sports clearances will be held August 11-14 and August 18-20
from 9 a.m. to noon in the HS Health Office.
- Schalmont Middle School:
The MS nurse will be available for fall 2008 sports
clearances Monday through Friday, August 4-15, from 8 a.m.
to noon during summer school. She also will have limited
hours available September 2-3 during staff development days.

Sports Physicals Dates (doctor
available):
- Schalmont High School
and Middle School
(grades 7-12): Students can come for a sports physical
on Tuesday, August 12, from 9-11 a.m. in the HS Health
Office.
- A physical from a student's
family doctor is acceptable and highly recommended.
