What is the Tdap
vaccine, and why must my child receive it?
According
to state law, all students entering sixth grade who are 11
years of age must have received a booster containing tetanus toxoids,
diptheria and acellular pertusis - known as the
Tdap
vaccine. Written proof of the vaccine is required to
attend school. The proof must be given to the Schalmont Middle
School nurse no later than 14 days after the first day of
school.
By when must my child receive the Tdap
vaccine?
The Tdap vaccine may be given any time after a child
has reached his or her 11th birthday. This means that parents
can take their children to a health care provider for the
vaccine well before the start of sixth grade. In fact, this
is highly recommended as the start of the school year can be a
very busy time. Written proof of the vaccine is required to

completed form must be given to the Schalmont Middle School
nurse no later than 14 days after the first day of school.
All students that are 10 years old when they start sixth grade
can wait until after their 11th birthday to get the Tdap
vaccine. At that time, they will be given a 14-day grace period
from their birthday to get the vaccine. Written proof of the
vaccine must be received by the MS nurse by the end of that

What documentation is required to prove
that my child has received the Tdap vaccine?
Written proof of the vaccine must be received by the
Schalmont Middle School nurse no later than 14 days after the
first day of school in September.
If the school nurse does not receive proof of a student's Tdap
vaccine, the student will not be allowed to attend school.
