Health Requirements
The following requirements apply based on a
student's grade level:

All students registering to
attend the high school are required to:
-
Have a recent physical examination
(within 12 months prior to commencement of school year).
Download a high school physical form.

Provide an immunization record
-
Have 3 doses or more of Diphtheria
Toxoid vaccine
-
Have 3 doses or more of Polio
vaccine
-
Have 2 doses of the MMR vaccine
-
Have 3 doses of Hepatitis B vaccine
-
As of 2010-11 school year, have 1 dose of the Tdap vaccine
(Tetanus Toxoids, Diptheria and Acellular Pertussis) if
the student is a ninth-grader
-
Have 1 dose of Varicella vaccine if
the student is a ninth- or 10th-grader
-
All ninth-graders are require to
have a scoliosis screening. Scoliosis screening is done by
the school nurse during physical education classes,
typically in February.
-
All new students and 10th-graders
are required by New York State Law to have a physical
examination. The exam is acceptable if administered not more
than 12 months prior to the commencement of the school year
in which the examination is required.
- All new students' and 10th-graders' health
certificates (e.g., physical examination, health appraisal)
must include an assessment of Body Mass Index (BMI) and a
determination of weight status category (based on BMI for
age percentile).
- All 10th-graders - Public schools must now
request a
dental health certificate
(PDF) from all students entering
entering kindergarten, second grade, fourth grade, seventh
grade, and 10th grade. A dental health assessment and the
certificate should be completed by the child's dentist.
The form should be returned in
person, by mail or by fax (355-5329) to the school nurse.
Health cards are handed out by homeroom
teachers in early September. These cards are very important
because they are used to keep teachers aware of students'
important medical information.

Health Requirements for Sports
Participation
The following requirements apply to all students who plan to participate in
sports:

Students must have a physical. This physical may
be completed by a student's family physician.
The school
physician conducts sports physicals in August. Generally
an interest meeting is held by the applicable sports coach,
and he/she hands out the necessary
sports packet.

Students must have a signed permission form.
They must also have an
interval health form when the most
recent physical date is more than 30 days from the start of
the applicable sports season. These sports forms should be
handed into the HS nurse prior to tryouts.

Dispensing of Medication in the School
Setting
In order for the school nurse to give
your child medication (prescription or over the counter) during
school hours, all of the following requirements must be met:

Student’s name

Name of medication
-
Prescribing doctor
-
Dose and time to be given
All medication must be hand delivered
to the school nurse or principal by a parent or responsible
adult. Students should not carry medications to and from
school.

Sick Students Policy
To insure the health of our students,
school policy regarding sick students is as follows:
-
A minimum of three doses of medication
must be administered before returning to school (Strep Throat
or Pink Eye).
-
Temperature has to be normal (98.6)
for 24 hours without medication before returning to school.

If your child is experiencing diarrhea
on the morning of school, they should remain at home for the
day.
-
Children who are experiencing vomiting
should remain at home until they are healthy enough to return
to school (preferably 24 hours after the last vomiting
episode).
