Mandatory: Physical Examinations and
Sports Clearance Physical examinations are REQUIRED for all students
participating in interscholastic sports. A private physical with
your own family physician is acceptable, and is highly
recommended. Your family physician is in the best position to
appraise your child's health, since he/she is most familiar with
your child's medical history.
The
physical examination forms you will need to take to your family physician
are available online.
All private physical forms are reviewed by the school nurse
before students can practice.

Want to play a spring sport?
Students must have an up-to-date physical on file in the
Schalmont Health Office, have their parents complete an
Interval Health History Form
(unless physical is completed within 30 days
prior to first day of practice), and come to one of the sports
clearance dates (see below) with their paperwork completed.
Spring 2012 Sports Clearance Dates
Student-athletes must bring their completed paperwork to the
high school health office on the designated date and time listed
below for their sport:
Thursday, Feb. 16, 2:15 p.m.
- Girls Softball
- Boys Tennis
- Baseball
Monday,
Feb. 27, 2:15 p.m.
Tuesday, Feb. 28, 2:15 p.m.
- Make-up date for all sports
Thursday, March 1, 2:15 p.m.
- Make-up date for all sports
