Mandatory: Physical Examinations and
Sports Clearance
Physical examinations are REQUIRED for all students
participating in interscholastic sports. A private physical with
your own family physician is acceptable, and is highly
recommended. Your family physician is in the best position to
appraise your child's health, since he/she is most familiar with
your child's medical history.
The
physical examination forms you will need to take to your family physician
now are available online. Click on the appropriate link below to download the form you'll need.
The documents are in PDF format, which requires Adobe Acrobat
Reader to view. Acrobat Reader can be downloaded for free from
the
Adobe Acrobat Web site.
All private physical forms are reviewed by the school nurse
before students can practice.
Want to play a fall sport?
Students must have an up-to-date physical on file in the
Schalmont Health Office, have their parents complete an Interval
Health History Form (unless physical is completed within 30 days
prior to first day of practice), and come to one of the sports
clearance dates (see below) with their paperwork completed.

Fall 2009 Sports Clearance Dates (nurse only):
- Schalmont High School (grades 9-12):
HS sports clearances will be held July 15-31
from 8:30 to 11:30 a.m. in the MS nurse's office due to
construction in the HS. If you can't come in person to see
the nurse:
- Fax the completed Interval Health Form to 355-5329
between July 15-24, or
- Mail the completed Interval Health Form before July 24

Dr., Schenectady, NY 12306
- Schalmont Middle School
(grades 7-8): MS sports clearances will be held
August 3-14 from 8 a.m. to noon in the MS nurse's
office. In addition, the MS nurse will be available
Tuesday, September 1 from 9 a.m. - 1 p.m. for any
student who has not handed in their paperwork. That will be
the last opportunity for fall sports clearance.

Fall 2009 Sports Physicals Dates (doctor
available):
- Schalmont High School
(grades 9-12): Students can come for a sports physical
on Wednesday, August 5, 2009, and Monday, August
10, 2009, from 8:30 - 10 a.m. in the Schalmont Middle
School cafeteria (a location change). Due to construction,
you can park in the MS parking lot.
- Schalmont Middle School
(grades 7-8): MS students can come for a sports physical
on Tuesday, August 4, at 1 p.m. in the MS nurse's
office. Due to construction, you can park in the MS parking
lot.
- A physical from a student's
family doctor is also acceptable and highly recommended.
